A 42nd-minute goal from Lautaro Martinez leveled the contest.
The result leaves River Plate alone atop Group E after the Argentine side beat Japan’s Urawa Red Diamonds 3-1 earlier on Tuesday in Seattle.
Ramos, the 39-year-old former Real Madrid star, rose to knock in a corner kick from Oliver Torres.
Inter Milan, coming off a blowout loss to Paris Saint-Germain in the UEFA Champions League final on May 31, pulled even on a well-worked set piece.
Kristjan Asllani chipped a free kick from the center of the pitch, 32 yards out, over the Monterrey defense in the 18-yard box. Inter’s Carlos Augusto ran onto the ball and squared a pass to Martinez for an easy tap-in.
Inter will oppose Urawa on Saturday in Seattle, while Monterrey will remain in Pasadena to face River Plate on Saturday.
